  • Last name
  • Bank account number of sender
  • Recipient’s full name and specific instructions
  • Receiving financial institution’s name
  • Receiving financial institution’s routing number
  • Receiving financial institution’s street address
  • The amount intended to transfer
  • Signature of sender
  • Other details as required
